If you are talking about NEET application form, then I will suggest you to correct it as soon as possible. It should be mentioned on the application form of AIIMS and JIPMER. So, you should make corrections soon so that you will not face any problem further.

As far as photo without name and date is concerned there is no such issue. It is okay to upload a photo without name and date. You can clear this by visiting the site and reading the FAQ section available on NEET website. All the best!

I would firstly advise you to make the correction as soon as possible. It should be displayed on the website of the examination. Be it NEET or AIIMS. Failing to make the corrections may provide grounds for barring you from appearing for the examination. The form should be filled with utmost accuracy and all necessary details must be provided. Note that the mistakes you make in the form may tantamount to serious problems later on.
