Get another application form and apply using it. There is no other way for it and if you lost the original copy then it won't be considered. If date is still valid and you can apply now also then apply it soon before the time lapse.

Documents from 10-12 should be taken care else it will cost you your career. hence forth keep all your documents in a file. don't keep it here and there and lose it.

Go and apply for it again soon..
