If while filling up the form it displayed there that you have input the correct details and the form accepted it then you do not have to worry about anything. There might be some trouble or glitch in the system due to which you could not get your photo and signature in the form. Keep trying to get the pdf. But when you are filling out an application it is only after you complete the process properly that you are allowed to download the application , so it means that you have filled it successfully. Try to download the pdf and it might come out properly.
