Neet phase -2 registration form is to be filled on official website of NTA -

https://neet.nta.nic.in

After the website opens, you have to click on - CORRECTION PHASE-II NEET UG. which you will see after you scroll the page down.

After which a tab would open where you will have to fill your application number, Date of birth and captcha after which you can proceed to your phase -2 registration.

It is good that you have already mailed NTA regarding your situation so that they can provide solution to your issue. And yes if unfortunately you are unable to fill your phase-2 registration then your result of NEET 2021 will not be declared.
