I assume your problem is that you forgot to add the surname in your application form. It is not like that your candidature will be cancelled by counselling officer at the time of documents verification. You can submit a self attested affidavit mentioning your fault supported by relevant documents and marksheets.

In most cases your problem will be resolved but the final decision will be of that officer only.
