Hi,
You have to write the result for 12th that you will pass in the form not for the failure or appearing might be an appropriate word. All the best!

Dear Ashutosh,

Mention the board you are appearing from for the 2nd time, the school you are appearing and fill up the form as it ask for the various queries, you need not mention about failing for the first time as they do not ask for it.
