I would advise you that to mail a undertaking to concerned authorities regarding your mistake and attach your fathers original identity card along with the undertaking . They will surely do some intitative to sort your problem as they dont this is very important step of your career .They will either unlock the form or will tell to submit your originals with undertaking at the time of verfication of documents
