Dear Vivek, PGIMER Chandigarh reserves 5% of the total seats for candidates who are working in rural areas. However, there are 2 conditions to be fulfilled. The poplulation should be lesser than 5000 and you should have been working atleast for 2 years. There is no format per se. You need to select the appropriate category while filling in the application form. During counselling and admission proces, you will have to submit the relevant documents as proof.
