The self declaration form given along with the admit card of the Neet exam  is enough to fill,it is given as a preventive measure in view of this Covid 19 pandemic.Most of the other exams like IBPS PO, Clerk etc...require an low exposure status in Arogya Setu application in the mobile.If in case a mobile not available filling the self declaration form will suffice.Best wishes
