There is no such option for you to fill sports quota in NEET apllication form. There is nno such quota provided on All India basis as such as well.

The application only has personal details, family details, academic details and files like photograph and signature to be uploaded. Besides, you will have to fill category details and choose a medium to answer the paper in and choose where you would want to give your exam.
