The answer to your question is No. If you have already applied for the application for JEE Mains 2021 phase 3, then there is no need to fill your form again. The application window was opened for those students who didnt apply earlier. If you have already applied to it, you would be getting your admit card soon.  JEE Mains phase 3 would be held from July 20 2021- July 25 2021.

If you want to appear for phase 4, you can apply it by using your login credentials. You need to edit your application form, add May session to it and pay the application fees. The application window for May session would be open from 9th to 12th July 2021 and exam would be held from 27th July to 2nd August 2021.
