Yes, you are eligible to write NEET exam. National Testing Agency release the NEET eligibility criteria for all medical aspiring candidates. As per eligibility criteria, the candidates aspiring for NEET should be 17 years as on December 31 of the admission year. and there is no upper age limit for students appearing in NEET. To be eligible you should have completed your 12th with PCB in with atleast 50% marks in aggregate if you are general category student.

Yes you are eligible for Neet 2021 examination with your date of birth

This is because  ,

There's no upper age limit for neet, there's Only minimum age limit according to which

to be eligible for neet 2021 one has to be at least 17 years old or more by  31st  December 2021 and you fulfil this criteria as you are more than 17 years old and thus you are eligible

Apart from age,
To be eligible you also must have passed in English and in both the theory and practical  of PCB i.e Physics, Chemistry and Biology with at least  50  % in aggregate of PCB ( in theory +practical) in 12th examination  if belong to general category , or   at least  40%  in aggregate of PCB if belongs to  Obc/ Sc /st .

So, if you fulfill this percentage requirement in PCB as per your category then you are definitely eligible for Neet 2021 examination, there will be no problem because of your age
