First of all at the time of physical reporting they will verify all your documents and they would check any disabilities if you have mentioned in the certificate. Since there can be many circumstances where you can gain weight or loose it even after mentioning in the certificate so it would not be a big issue unless your original weight and weight in medical certificate differ a lot.
