Dear Asha, the only change that is happening to NEET 2020 is that even AIIMS and JIPMER will conduct their admissions through NEET instead of their respective entrance examinations. Rest all remains the same. Regarding whether the paper would be tough or easy depends on how you prepare for the exam. NTA is known to give questions from NCERT books. It has got nothing to do with AIIMS and JIPMER conducting their admissions through NEET. We cannot predict the difficulty of the paper until the exam has been over. Anything we say at this point will be purely an assumption and not a fact.

Yes.. cbse used to conduct NEEt examinations till 2018 and when NTA conducted the same in 2019,the cutoffs went drastically high as much as almost 50-60 marks more than the previous year. The competition went very high as the number of candidates had increased and also the paper level was called easy comparatively. This year entrance to AIIMS and such courses will all be done through neet only. So yes the competition will surely increase. Even the exam might get conducted twice a year even though there hasnt been an official notification yet.
Dont think of the competition and everything right now, as you are a repeater, you have a upper hand as you don't have to prepare for boards exams and also you have been through portion once. So keep preparing well, stay focused and achieve things.
