NEET 2021 application form is not released yet.  Neet application form is expected to be released by the end of January or early february 2021. You will be able to fill the application form on ntaneet.nic.in once released. You should keep following documents ready for filling NEET application:

• Mobile Number
• Email address
• Class 10th and 12th marksheet
• Scanned images of passport-sized photograph and signature
• Aadhar Number
• Scanned Left-hand thumb impression
• Any other docs (if applicable)
