Yes , you can wear t-shirt in the photograph which you are going to upload in your neet application form. You need to upload two photos which is given below with their specifications

Passport size photograph "- It should be coloured with white background , preferably with name and date of taking the photograph,  with no cap or goggles, 80% face coverage should be there in the photo, ears clearly visible in your photo

Postcard size picture:- Your post card picture should be taken on or after 1st September 2019, preferably with name and date of taking the photograph , you should not be wearing cap or goggles in your post card size  photograph.
