You have to go to the JOSAA official website. I cannot upload the exact pdf document due to company norms.

When you go to the website, go to Business Rules and download the pdf. There on page 75 you will see Annexure 8 which is the format for the medical certificate.

No, it is not necessary to have it signed by government doctors. Any Registered Medical Practitioner can do the same for you.
