A self declaration form is a declaration made by candidate that he/she is eligible for the post as all the conditions kept forward by the institute is fulfilled by him/her.

The institute usually provides a format in which the form needs to be submitted and as mentioned by you that you have got an undertaking form along with card itself. So, there is no need to take a separate self declaration form unless asked by the institute.
