Most probably there will be no problem for you in the counselling. It must be from component authority and it's contain all the necessary information mentioned in that with proper sign of issuing authority. So don't worry much about the given format.  It's should be valid caste certificate only.

Otherwise if you can issue it in the given format it would be best. Then also you don't need to worry much about it.
