During the time of correction you can correct the name as your 10 th marksheet.

Go with the details as per your 10th marksheet. You can change your details during the correction of application form. You will be allowed once to correct your details. Do it carefully. Good luck ! 

You need to consider the details which are there in your 10th documents and accordingly you will have to the application form for NEET 2018.
