You need undertaking form only when you are applying for undergraduate course. You can check in the upload documents section if it is compulsory to upload or not. If it is then you can follow the given process.

You can download this undertaking form from the Mumbai University website, take a print out fill it out and then upload it on the H.R college website in the upload documents section.

You would have to fill in your personal details, course, college which you are applying to and your university digital portal.

You can check the instructions manual to find the undertaking form it is available at the University website also on the college website. If you are still unable to find it you can mail to the official email provided by the college.
