For the NEET-UG Counseling, a PwD (Person with Disability) Certificate is a necessary document. In order to claim their PwD Quota in the NEET-UG Counseling, students who qualify for PwD Quota Seats must present a PwD Certificate from the relevant Authority. To obtain your PwD Certificate, you must first download the Pro Forma (Form) of PwD Certificate from the NTA NEET-UG Prospectus, which is located on Page No. 71 Appendix IX. You may also download that Form from the Counselling Scheme Brochure of the MCC Counselling website. Once you have completed the Form, you must go to one of the hospitals listed below to pick up your certificate, which will then be signed and attested by the appropriate specialist.

You may visit any hospital on this list that has been authorised by NTA to issue your PwD certificate:-

1. Vardhman Mahavir Medical College & Safdarjung Hospital, Delhi
2. Institute of Medical Sciences, Banaras Hindu University, Varanasi
3. Govt. Medical College and Hospital, Sector 32, Chandigarh
4. SMS Medical College, Jaipur
5. All India Institute of Physical Medicine and Rehabilitation, (for locomotor disability only) Mumbai.
6. Institute of Post Graduate Medical Education & Research, Kolkata
7. Madras Medical College, Chennai
8. Grant Medical College, J. J. Hospital, Mumbai
9. Goa Medical College, Goa
10. Government Medical College Thiruvananthpuram, Kerala
11. JIPMER, Puducherry
12. Govt Medical College, Agartala

Note : To receive your PwD Certificate, you can go to any of the hospitals mentioned above. Don't forget to download the form from the NTA NEET-UG Brochure or the Counseling Scheme Brochure, all of which are available on the MCC website.
