Now coming to your query, Documents required for PWD certificate are Copy of Address proof(Aadhar/Driving License/State Domicile, etc), Copy of Identity proof (Aadhar card/Driving License/Voter Card/Pan Card, etc), Recent Photo, etc. Along with documents you need to undergo Medical check-up in one among the following 4 centers:

                              1. All India Institute of Physical Medicine and Rehabilitation. Hazi Ali Park, K.Khadya Marg, Mumbai.

                              2. Institute of Post Graduate Medical Education and Research, 244 Acharya J.C. Bose Marg, Kolkata.

                              3. Vardhman Mahavir Medical College and Safdarjang Hospital, Ansari Nagar, Delhi.

                              4. Madras Medical College Park Town, Chennai.

Regarding UDID there is no official information.
