It is required in every college during admission process or counselling process, to getting that certificate you can go to any Govt certified doctor who is near by to your home or locality ,they will examine you and make sure that you are fit or not to attend college on regular basis , mostly such certificates are required if any student is suffering from any chronic health illness, so that certificate will be duly filled by doctor and should be submitted during admission process!

Medical fitness is a proof for college about your fitness level of the candidate. It's compulsory to submit the medical certificate during admission procedure from a recognised doctor or good medical practitioner.

Making of medical certificate takes time. So, i recommend you to make it before.
