Cbse Will notify it in their website... it's a easy procedure where u have to fill an application form ....
You can revaluate a maximum of 4 subjects(excluding optional subjects).

You will have to request CBSE for re evaluation. You can ask your school authorities for the process of the same.
It can allowed for a subject or 2 but not  more than that.
