Check out the information brochure which has the instructions to fill the details. There must be some instruction with examples what to fill in entry number.

If it is about certificate number then there must be some code or registration number at the top part of the respective  certificate. Please check out and fill the details. Make sure not to make any mistake as it is very hard to make correction. If not your form will get rejected.
