You need to choose the code 2.

This is because,

Though you are doing bpt your highest qualification with all the three subjects i. e physics chemistry and Biology/ biotechnology remains your 10+2

While, those who have passed 1st year of 3 yeargraduation with all the three subject i. e physics chemistry and Biology/biotechnology needs to select code 5 and those who have completed BSc with at least any two of physics chemistry and Biology in their bsc need to select code 6.

Here's a short brief of Neet codes :-

--------) Code 1 :- code 1 is for class 12th board 2021 examination appearing students with physics, chemistry, biology and English or have have already appeared but results are awaited i. e have not been declared.

--------) Code 2 :- code 2 is for all those who have passed their 10+2/higher /senior secondary/ Indian School Certificate Examination with physics, chemistry, biology and English

--------) Code 3 :- code 3 says it is for those passed their intermediate / pre- degree examination with physics, chemistry, biology/biotechnology and English ,your physics, chemistry and biology/biotechnology should also contain practical exam.

--------) Code 4 :- pre - medical degree with Physics, chemistry and biology/biotechnology after passing hsc i. e 10+2 or puc or equivalent.

-------) Code 5 :- code 5 is for all those who have passed 1 st year of 3 yeargraduation with all the three of Physics, chemistry and biology/biotechnology

--------) Code 6 :- code 6 is for all those students who have passed BSc with at least any two of following subjects :-

--------) Code 7 :- code 7 is for those who have passed their examination from foreign board which is equivalent of Indian 10+2 board.

This is because the last time you studied physics, chemistry and biology was in class 12th. So you will be considered as candidate who has cleared 10+2 with science stream.
