You can find the self declaration form for TS EAMCET 2021 after downloading the admit card. You can get the self declation form on official website of TS EAMCET.

The COVID 19 self declaration form has to be filled before entering the exam hall. You will have to mention your personal details, hall ticket number, exam date and other details in the self declaration form. You also need to provide details about COVID-19 symptoms in the TS EAMCET self declaration form. If you have any of the symptoms you will be isolated from other students.

Along with this, you also need to carry your Hall ticket and photo ID proof. You can carry your personal masks and hand sanitizer.
Reach the exam center 2 hours before exam to avoid over crowding.
