These days application forms for almost all entrance exams are done through online mode. A lot of people forget their login details so there is always an option for recovery of details.

In your case, you can use the forgot password option to recover your password through your email id. Alternatively you can go through your inbox and you would have received a mail from the authorities after initial registration and you could get application number from there.
