As per the given information, you must have filled wrong name and DOB which is not as per your class 10 marksheet, so you have to make corrections in your form.

ICAI has made available the online error correction window facility for the students who have committed errors or mistakes while filling their form.

You have to make corrections as asked by them to do so via this message.
