It is unlikely to get a government MBBS seat in a top medical college with a score of 490 in NEET, especially for the general category. The competition is very high, and cutoffs for government colleges are generally higher than 550.

However, depending on the state and category (especially reserved categories like SC, ST, and OBC), it might be possible to get a seat in a government medical college with a score around 490.

If you are not keen on repeating the NEET exam, you could explore other medical courses like BDS (Bachelor of Dental Surgery), BAMS (Bachelor of Ayurvedic Medicine and Surgery), or BHMS (Bachelor of Homeopathic Medicine and Surgery), which might have lower cutoffs.
