For counselling, you don't need to carry the application form. You need to carry your Admit Card, Academic Certificates, Passport-size Photographs, Work Experience certificates (if any), Caste Certificate (if applicable), and any other document if mentioned.

As a proof there needs to be something at time of counselling so its better to go with printout of application form. It may be required at time of counselling.
