Ideally, MCI regulated that the disability certificate should be from any of the disability assessment centres. The list is available at MCC official website. But now you can get the certificate from a District Medical College but the assessment has to be done by a team of Doctors from the Disability Assessment Centers. You can get examined at any Government Medical College/ District Hospital/Government Hospital. Such Institutions shall issue a Disability certificate. After selection under PWD Category, candidates should produce a Disability certificate issued by the Disability Assessment Board. The DAB eill assess the Candidate in reference with the criteria prescribed by NMC. So once the results have been declared, it is important to be assessed by the DAB. If the DAB considers the Candidate ineligible for PWD reservation, such Candidates cannot register for Counselling. The list of the DAB centres is listed in Page no. 72 at: https://ntaneet.nic.in/Ntaneet/ShowPdf.aspx?Type=50C9E8D5FC98727B4BBC93CF5D64A68DB647F04F&ID=AC3478D69A3C81FA62E60F5C3696165A4E5E6AC4

Hello dear !
Greetings!
First of all I want to tell you that as per the latest NEET(UG)-2020 information brochure , a total of 5% of reservation is provided to physically handicapped aspirant.
There are various guidelines and criteria under which a physically handicapped candidate gets 5% reservation under pwd quota (person with disability) .
As per mci latest circular for neet-2020 , locomotive disability of limbs is included in criteria for pwd quota.
But the main important thing to note is the locomotive disability must be 40%-80% for the candidate to be eligible to get admitted in a medical course under pwd quota through neet -2020 .
This assesment would be done by medical board of governers at specified centres by mci which i am listing down here
  1. Vardhman mahaveer medical college and safdarganj hospital , new delhi
  2. All india institute of physical medicine and rehabilitation (for locomotor disability only) , hazi ali , mumbai
  3. Institute of post graduate medical education and research , kolkata
  4. Madras medical college , park town , chennai
  5. Goa medical college , tiswadi , goa
  6. Sms medical college , jaipur , rajasthan
  7. Institute of medical sciences , banaras hindu university , varanasi , uttar pradesh
So you have to verify your disability at one of these centres and get your ph certificate.
Afterwards you will be eligible for admission under pwd quota if your disability is assesed to be between 40%-80%.
Thanku , feel free to ask any doubt
