1- COMEDK RANK CARD : Original Printout +2 separate set of photocopies 

2- COMEDK ONLINE APPLICATION FORM : Original Printout +2 separate set of photocopies 

3- COMEDK TAT THAT WAS USED FOR THE EXAM : Original Printout +2 separate set of photocopies

4- CANDIDATE ORIGINAL ID : Original +2 separate set of photocopies

5- PARENT/ GUARDIAN'S ORIGINAL ID PROOF (ACCOMPANYING THE CANDIDATE)  : Original +2 separate set of photocopies

6- BIRTHDATE PROOF OF CANDIDATE : Original +2 separate set of photocopies

7- 12TH STANDARD OR EQUIVALENT EXAMINATION CERTIFICATE : Original +2 separate set of photocopies

8- DEMAND DRAFT OF RS. 55000 FROM APPROVED LIST OF BANKS IN THE FORMAT REQUIRED BY COMEDK :  Original +2 separate set of photocopies

Karnataka domicile certificate (Issued by Revenue authority, not below the rank of Tahsildar) (If applicable)
