The centres are allocated first of all in the exam city you choose. The centres then are randomly allocated, most often based on the application number, which means if you and your friend have registered simultaneously and applied for same exam city with almost same time payment and application status, you have good chances of getting the same exam centre. This was the scenario when I gave my AIIMS exam in 2016.
The system and algorithms might have changed, but I guess it’s based still on the application number.
