Dear Nehal, every year a different IIM conducts CAT. This year it is IIM Kozikhode. More than filling the form according to last year's form, you should fill in the form based on current and accurate data. If you belong to OBC-NCL, then you should fill the form accordingly. On the official pagr, to the right bottom of the website, you will find Caste certificates for every Category in a pdf format. You need to download that form, fill in the details, self attest it and upload it to your application form as a proof of your Category.

Dear aspirant, every year new form is there and the last year from is considered for the admissions at that time only, this year you can fill the form as per the details and provide the certificate if required.
