From where we can get medical fitness certificate for neet counciling. I m from hapur district in up.

Dear Sabir, the form for medical fitness certificate is generated by the respective state authorities and you need to get filled it by any govt/private doc who has valid licence of practicing medicine and has his own stamp.

Any doctor who has equivalent degree to MBBS like BDS is also acceptable.

The Medical Fitness form is generally attached at the end of the PDF issued by the Health University of State Website.

Even if it's not available there, a link would soon be made available to download it and get it printed.

Accordingly, make sure to get yourself thoroughly assessed by the doctor performing the required tests and attaching the reports too.
