If you got 490 marks in NEET and you are from the General category, then getting admission in a government MBBS college is very difficult. Most government colleges need at least 550 to 600 marks. But you have a good chance of getting into a private MBBS college, especially through management quota. Some deemed universities may also accept students with your score. If your state has lower cutoffs, you might get a seat in a private college through the state quota. You should take part in both All India and State counselling and keep checking for private or deemed colleges. You can still get an MBBS seat, just not in a top government college.
