There are some condition you must know about photo uploading to any online form are:

• Most likely the size of the photo is too small. Your photo must be at least 100kb in size or it can be of any size as they demand for it.
• It might be your photo is not a JPEG, PNG or GIF.As they may ask for a particular extension photo to be upload

So, here I would suggest you to to form and collect information about size or format the ask for.
