An identification marks helps to verify a person if he/she is the candidate who has applied for a particular application. Identification marks can be mole, birth marks or scars. You may find a mole or birth mark your body and fill it in the application form. For example if you have mole of your right elbow then you can mention that there is 'A mole on right elbow'. We would suggest you to not mention the scars because they might fade away over the time. Ensure that you mention the moles which are visible even when wearing clothes such moles on face, neck, palm, hands, arms, ears etc.
