The procedure for Issue of Ward Certificate-

The candidates need to approach the requisite RO/SRO after the issue of admission Notice in Pubic domain by ESIC, HQ office with an application form and self attested copies of the documents to be varified from the Originals given below

ESIC Pehchan Card

NEET Admit Card containing Roll No.

Proof of Residence

School Certificate for proof of Date Birth/ Birth Certificate

Senior Secondry School certificate
