For mbbs you need to fill NEET form. Procedure is mentioned below-

1. Go the official portal of the NTA conducting authority (Link given above).
2. Click on NEET UG 2019 link.
3. Click on “Apply Online” link.
4. Read all the instruction and then click on “Proceed to Apply Online NEET (UG) 2019”.

Documents required are-

1. 10th class marksheet
2. 12th class marksheet
3. Scanned copy of candidate’s passport sized photograph
4. Scanned copy of candidate’s signature
5. Valid ID proof
