It is always advisable to get a medical certificate from a Registered Medical Practitioner or any Govt. hospital . Such a certificate should be signed by a Medical Board in the case of a Gazetted Government servant and by a Civil Surgeon or a District Medical Officer or a Medical Officer of equivalent status in the case of a non-gazetted Government servant other than Class IV.  You may approach private doctors too , but a certificate signed by a government doctor is considered more valid for all purpose,you may check that up with the organization where you need to submit it to be more clear and follow their specific guidelines if any.
