With your marks of 491/720 in NEET 2019 and considering you are from the unreserved category (because you have not mentioned your category in the question) your All India Rank will be anywhere in the range of 24,000-26,000. This means that you're in a good position to get into a government college in West Bengal under the 85% State Quota as last year's cut-offs were for Murshidabad Medical College and Hospital, Murshidabad with the rank of 29395 with the marks 481. There is a narrow margin but do try your luck.
