Dear Nagaveni, if you want the EWS certificate in your name, you should be writing your name. Though you are a minor, because it will also be used for Academic purposes, you should write your name. Your father's name will have to be written under Parent's column.

in the counciling time will they ask my fathers ews certificate or my ews certificate. I am of 16 years old
