I'm afraid there will be negative implications of this, and no, you cannot change it once you submitted the form and the date had passed.

You're best bet is to go to the document verification with all your original documents, plus an affidavit from the local notary.

In the affidavit, you must state that the discrepancy in the form was from your side, and the correct details are in the document you carry.
