If I consider the previous year data .Then you have mild chances of getting admission in any of the government colleges in madhya pradesh through state quota .Previous year minimum marks requiere for a general candidate was 493 Government Medical College, Khandwa.which is very near to your score.
The cutoff changes almost every year,so nothing can be predicted much.cutoff depends on the difficulty level of the paper and the number of students who appeared for the entrance exam.

I think that you are talking about the NEET exam. With a marks of 499 with out a reservation it is mediocre chances for you to get a government college in NEET MBBS course. But my suggestion is that you go for counselling and try you best until the last round of the counselling and your chances will increase with every round.
