Is my ews certificate is applicable for my father also in hospital can u tell me it's urgent

Yes, You will need to fill both your name as well as your fathers name in the EWS certificate.

Economically weaker section under general category has to submit an income and assets certificate which must be attested by the higher authorities either -

• District magistrate, Additional district magistrate, collector, deputy commissioner, additional deputy commissioner, sub-divisional magistrate, taluka magistrate extra assisstant commissioner, Chief presidency magistrate, presidency magistrate.
• Revenue officer not below the rank of the tehsildar. 
• Sub divisional officer of area where candidate and /or his family resides.

These are the ones who have the authority to attest income and asset certificate of Economically weaker sections, belonging to general category. 

10% reservation quota is mandatory for EWS of general category . Once you see any application form of entrance exam or any websites of a particular college and you apply under EWS quaota, then you need to produce this certificate. Often you will find this in appendix or at the end of the information brochure while downloading the Information brochure. 

Here, the certificate is of you claiming for reservation under EWS , hence you will have to give your name first followed by son / daughter or wife's name and your home address and in which final year you are applying for the year should be mentioned. This particular appendix is the same throught exams and admissions etc. 

Also in order to claim it you have to potentially agree that  you 

• dont own 5 acres of agricultural land and above
• family annual income is not more than 8 lakhs
• dont have any residential flat of 1000sq ft and above
• don't have residential flat of 100 sq. yards and above in notified municipalities and 200 sq. yards and above for non notified municipalities. 

Also along with that you need to give confirmation that you belong to the caste which excludes from SC/ST/OBC and mention your name in space provided. 

You need to affix a photo which is recent passpoert size photos. Later on the authority will sign the certificate, attest it and get the certificate from them. This will be vaild for that particular year. 

By choosing so all this you need to apply and submit the certificate while counselling or at time of admission. 

So you will have both your father and your name in the certificate. 

There are precisely other condition on what they call a 'Family" as while considering EWS. For that to know you need to read the terms and conditions. 

There is 10% reservation for EWS which is getting mandatory .
