address - yeah can change easily, domicile certificate will be required if got admission in your native state by state quota.

category - if filled general behalf of obc/st/sc bring your caste certificate you will change your category not easily but yeah can change. 

You can change the address but for category, you will need the proper category certificate and a strong affidavit ( signed by district megistrate) then you may have some chances if any.
