You must be aware of the fact that diffrent examinations and diffrent colleges have different guidelines regarding the application form, so until and unless you don't mention the name of the exam/College, we won't be able to provide specific information. 

Generally speaking, irrespective of the examination or College, if you have uploaded a photo with both date and name, it must not create any particular problem.
